I am having a problem with my Remington Magazine Extension. The bolt that attaches to the 870's tubular magazine comes loose. I torque it down all the way and can hear the dimple in the bolt engage the ball on the hand guard. I use the standard barrel clamp. Yet when I fire 15-20 rounds the bolt loosens up.
Any thoughts and or suggestions would be appreciated.

Not quite clear on your terminology. Just so we're on the same page, the long element that actually provides space for the extra shells is the extension tube. The internally-threaded part that connects the extension tube to the magazine tube (which is brazed to the receiver) is the coupling. The coupling tightens down against the barrel guide ring, which is the cylindrical element that fits over the magazine tube and is brazed to the barrel. In the front face of the barrel guide ring is a small, spring-loaded metal part called a magazine cap detent that clicks into notches in the underside of the coupling to help keep it in place. Finally, the extension bracket is what clamps to both the barrel and the extension tube a few inches back from the front end of the extension tube.
Now, what it sounds like is happening, is that either your coupling or the extension as a whole is not staying tight against the barrel guide ring under recoil. The first thing to check is your installation. The factory Remington extensions should be installed one piece at a time to ensure that they fit & function correctly. Scroll down in this review for a detailed installation guide with pictures. Make sure you get the bracket screw good and tight; if it seems like it's coming lose even when tightened down hard, you could try using some blue threadlocker.
